Understanding Anticoagulant Medications for NCLEX Success
Anticoagulant medications are a high-yield NCLEX topic. These drugs prevent clot formation and are essential in managing conditions like DVT, PE, atrial fibrillation, and mechanical heart valves.
Heparin (Unfractionated)
Heparin works by activating antithrombin III, which inhibits thrombin and factor Xa. It is administered IV or subcutaneously and monitored via aPTT (activated partial thromboplastin time). The therapeutic range is typically 1.5 to 2.5 times the control value. The antidote is protamine sulfate. Key nursing considerations include monitoring for heparin-induced thrombocytopenia (HIT) and avoiding IM injections.
Warfarin (Coumadin)
Warfarin inhibits vitamin K-dependent clotting factors (II, VII, IX, X). It is taken orally and monitored via PT/INR. The therapeutic INR range is 2.0 to 3.0 for most conditions (2.5 to 3.5 for mechanical heart valves). The antidote is vitamin K (phytonadione). Teach patients to maintain consistent vitamin K intake and avoid foods that drastically change their diet. Warfarin has numerous drug interactions including NSAIDs, antibiotics, and herbal supplements.
Direct Oral Anticoagulants (DOACs)
DOACs include rivaroxaban, apixaban, edoxaban (factor Xa inhibitors), and dabigatran (direct thrombin inhibitor). These medications do not require routine lab monitoring. Dabigatran has a specific reversal agent called idarucizumab (Praxbind). DOACs have fewer food interactions than warfarin but still carry bleeding risk. Teach patients to report unusual bleeding, dark stools, or blood in urine.
NCLEX Priority Nursing Interventions
For all anticoagulants, assess for signs of bleeding including petechiae, ecchymosis, hematuria, melena, and gingival bleeding. Use soft toothbrushes and electric razors. Hold pressure on injection sites for at least 5 minutes. Never give aspirin concurrently without a provider order.
