Cardiac Content on the NCLEX: What You Need to Know
Cardiac nursing is one of the most heavily tested content areas on the NCLEX-RN. Questions frequently involve interpreting electrocardiogram rhythms, managing heart failure exacerbations, recognizing myocardial infarction presentations, and implementing emergency interventions. The exam tests not only your factual knowledge but your ability to prioritize nursing actions, recognize life-threatening changes, and anticipate complications in cardiac patients. Building a strong cardiac foundation significantly improves your performance across multiple content areas because cardiovascular principles intersect with pharmacology, fluid and electrolyte balance, and critical care nursing.
Essential ECG Interpretation
Normal Sinus Rhythm
Normal sinus rhythm originates from the sinoatrial (SA) node at a rate of 60 to 100 beats per minute with a regular rhythm. Each QRS complex is preceded by a P wave, and the PR interval falls between 0.12 and 0.20 seconds. The QRS complex should be narrow, measuring less than 0.12 seconds. Understanding normal sinus rhythm is essential because it serves as your baseline for identifying every dysrhythmia.
Critical Dysrhythmias
Ventricular fibrillation (V-fib) produces chaotic, disorganized electrical activity with no identifiable P waves, QRS complexes, or T waves. There is no cardiac output during V-fib, making it a pulseless rhythm requiring immediate defibrillation and CPR. Ventricular tachycardia (V-tach) presents as wide, bizarre QRS complexes at a rate exceeding 150 beats per minute. Pulseless V-tach is treated identically to V-fib. If the patient has a pulse with stable hemodynamics, antiarrhythmic medications such as amiodarone are administered.
Third-degree (complete) heart block occurs when atrial and ventricular rhythms are completely independent. The atrial rate is typically normal while the ventricular rate is dangerously slow, often 20 to 40 beats per minute. Heart Failure Management
Heart failure questions on the NCLEX require understanding the difference between left-sided and right-sided failure. Left-sided heart failure produces pulmonary symptoms: dyspnea, orthopnea, crackles on auscultation, and pink frothy sputum in severe cases. Right-sided heart failure causes systemic venous congestion: jugular venous distention, peripheral edema, hepatomegaly, and weight gain.
Nursing management of heart failure focuses on reducing cardiac workload through positioning (high Fowler's), administering prescribed diuretics (typically furosemide), monitoring daily weights (a gain of two or more pounds overnight indicates fluid retention), restricting sodium intake to less than 2,000 milligrams per day, and monitoring intake and output. Teach patients to weigh themselves at the same time each morning using the same scale and to report weight gains exceeding two pounds in one day or five pounds in one week.
Key Medications in Heart Failure
ACE inhibitors reduce afterload and are a cornerstone of heart failure treatment. Digoxin increases contractility and slows heart rate; monitor for toxicity signs including nausea, visual disturbances (yellow-green halos), and bradycardia. Check the apical pulse for one full minute before administering digoxin, and hold the medication if the rate is below 60 beats per minute. Acute Coronary Syndrome
Acute coronary syndrome encompasses unstable angina and myocardial infarction (MI). Classic MI symptoms include substernal chest pain radiating to the left arm or jaw, diaphoresis, nausea, and shortness of breath. Women, elderly patients, and individuals with diabetes may present atypically with fatigue, epigastric discomfort, or dyspnea without chest pain. The mnemonic MONA (Morphine, Oxygen, Nitroglycerin, Aspirin) guides initial intervention, though current guidelines emphasize aspirin administration and obtaining a 12-lead ECG within 10 minutes of presentation. Oxygen is now administered only if the SpO2 falls below 94%.
