Elder Abuse Assessment and Reporting for the NCLEX

Types of Elder Abuse

• Physical: Unexplained bruises, burns, fractures, especially in various stages of healing
• Emotional/Psychological: Withdrawal, depression, fearfulness, changes in behavior
• Financial: Sudden changes in finances, missing belongings, unexplained withdrawals
• Neglect: Poor hygiene, malnutrition, dehydration, untreated medical conditions, pressure injuries
• Sexual: Bruising on inner thighs, genital area; unexplained STIs

Red Flags

• Inconsistent explanations for injuries
• Caregiver answers all questions, doesn't allow private conversation
• Client appears fearful of caregiver
• Delay in seeking medical treatment
• Frequent ER visits with vague complaints

Nursing Responsibilities

• Nurses are mandatory reporters - report suspected abuse, even without proof
• Interview the client alone (without suspected abuser present)
• Document objectively: exact quotes, detailed description of injuries, photos if permitted
• Report to Adult Protective Services (APS)
• Do NOT confront the suspected abuser
• Ensure client safety before discharge
