Understanding Bowtie Items on the NextGen NCLEX
Bowtie items are one of the newest question types on the NextGen NCLEX. Named for their visual shape resembling a bowtie, these items test your ability to connect clinical data, conditions, and nursing actions in an integrated format.
Bowtie Item Structure
A bowtie item has three columns. The center column contains the patient condition or diagnosis you need to identify. The left column lists clinical findings or assessment data that support the condition. The right column lists nursing actions or interventions appropriate for the condition. You must correctly identify the condition in the center and then select the relevant cues on the left and appropriate actions on the right.
How Bowtie Items Are Scored
Bowtie items use a partial credit scoring model, meaning you receive credit for each correct selection you make. Unlike traditional multiple-choice questions with all-or-nothing scoring, you can earn partial credit even if you do not get every selection correct. However, incorrect selections may reduce your score within that item. This scoring model rewards partial knowledge while still differentiating higher-performing candidates.
Strategy for Bowtie Items
Start with the center column. Read the clinical scenario carefully and identify the most likely condition based on the presenting data. Then work outward to each side. For the left column, select the assessment findings that are most directly related to and indicative of the condition you identified. For the right column, choose interventions that are appropriate and prioritized for that specific condition. Avoid selecting interventions that sound generally correct but are not specific to the identified condition.
Practice Tips
When studying, practice connecting the three elements: What data supports this diagnosis? What is the most likely diagnosis? What are the priority nursing actions?
