IV Therapy Fundamentals

Understand IV solution types and their uses. Isotonic solutions (0.9% NS, lactated Ringer, D5W) expand intravascular volume without shifting fluid between compartments. Use for dehydration and blood loss. Hypotonic solutions (0.45% NS) move fluid from the vascular space into cells. Use for cellular dehydration. Do not use in patients with increased ICP or third-spacing. Hypertonic solutions (3% NS, D10W) pull fluid from cells into the vascular space. Use cautiously for severe hyponatremia. Monitor for fluid overload.

Total Parenteral Nutrition (TPN)

TPN provides complete nutrition intravenously for patients who cannot eat (bowel obstruction, severe pancreatitis, major surgery, short bowel syndrome). TPN contains dextrose, amino acids, lipids, electrolytes, vitamins, and minerals. It must be administered through a central venous catheter due to high osmolarity. Nursing considerations include monitoring blood glucose every 6 hours (hyperglycemia is common due to high dextrose content), using an infusion pump for precise delivery, never abruptly discontinuing TPN (taper to prevent rebound hypoglycemia), changing tubing and filter every 24 hours, monitoring liver function and triglycerides, and assessing for signs of infection at the central line site.

Medication Safety Principles

Follow the rights of medication administration for every medication. High-alert medications require independent double-checks by two nurses. These include insulin, heparin, warfarin, chemotherapy, opioids, and neuromuscular blocking agents. Use tall man lettering to distinguish look-alike/sound-alike medications (for example, hydrALAZINE vs. hydrOXYzine). Report medication errors through the facility reporting system and participate in root cause analysis.

Adverse Drug Reactions

Monitor for allergic reactions (rash, urticaria, anaphylaxis), idiosyncratic reactions, drug-drug interactions, and drug-food interactions. Document and report all adverse reactions. Know which medications require monitoring of serum drug levels including digoxin, lithium, aminoglycosides, vancomycin, and phenytoin.
