Physiological Integrity: Exam Strategies
Physiological Integrity is the largest category on the NCLEX-RN, covering pharmacology, reduction of risk potential, basic care, and physiological adaptation. Master these strategies to handle the most content-heavy portion of the exam.
1. Critical Lab Values You Must Know
The NCLEX expects you to interpret lab values and act on abnormal findings. Memorize these high-yield reference ranges:
- Potassium (K+): 3.5-5.0 mEq/L. Hypokalemia causes muscle weakness, cardiac dysrhythmias (flattened T waves, U waves). Hyperkalemia causes peaked T waves, bradycardia, and risk of cardiac arrest.
- Sodium (Na+): 136-145 mEq/L. Hyponatremia causes confusion and seizures. Hypernatremia causes thirst, dry mucous membranes, and restlessness.
- Blood glucose: fasting 70-100 mg/dL. Hypoglycemia (below 70): cool, clammy, tremor, confusion -- treat with 15 g fast-acting carbohydrate. Hyperglycemia: polyuria, polydipsia, polyphagia.
- INR: therapeutic range on warfarin is 2.0-3.0 (2.5-3.5 for mechanical heart valves). Above range = bleeding risk.
- Digoxin: therapeutic range 0.5-2.0 ng/mL. Check apical pulse for 1 full minute before administration; hold if below 60 bpm in adults.
- BUN/Creatinine: BUN 10-20 mg/dL, Creatinine 0.7-1.3 mg/dL. Elevated values indicate impaired renal function -- question nephrotoxic medications.
- WBC: 5,000-10,000/mm3. Elevated suggests infection; below 5,000 indicates neutropenia and infection risk.
- Hemoglobin/Hematocrit: Hgb 12-16 g/dL (female), 14-18 g/dL (male). Low values may indicate anemia or hemorrhage.
2. Medication Calculation Strategies
Dosage calculation questions require accuracy. Use dimensional analysis (factor-label method) as your primary approach:
- Write what you want on the left side of the equation.
- Write what you have as your starting factor.
- Set up conversion factors so units cancel until you reach the desired unit.
- Multiply across numerators, multiply across denominators, then divide.
Common calculation types on the NCLEX:
- IV drip rates: mL/hr or gtt/min. Formula: (Volume x Drop factor) / Time in minutes = gtt/min.
- Weight-based dosing: Convert pounds to kilograms (divide by 2.2), then multiply by the dose per kg.
- Pediatric safe dose range: Calculate the safe range using the child's weight, then compare the ordered dose. If it falls outside the range, do not administer and notify the provider.
3. Recognize Complications Early
Many NCLEX questions describe a clinical scenario and ask which finding requires immediate action. Train yourself to recognize these critical complications:
- Pulmonary embolism: sudden dyspnea, pleuritic chest pain, tachycardia, anxiety, low SpO2.
- Compartment syndrome: pain out of proportion to injury, pain with passive stretch, paresthesia, pallor, pulselessness (late sign). This is an emergency.
- Increased intracranial pressure: decreased level of consciousness, Cushing's triad (hypertension, bradycardia, irregular respirations), unequal pupils.
- Autonomic dysreflexia (spinal cord injury above T6): sudden severe hypertension, pounding headache, bradycardia, flushing above the level of injury. Sit the client up and find the trigger (usually bladder distension).
- Transfusion reaction: fever, chills, flank pain, hives, dyspnea. Stop the transfusion immediately, keep the line open with normal saline, and notify the provider.
4. Assessment vs. Intervention: Which Comes First?
A common NCLEX trap is choosing between assessing and intervening. Use these guidelines:
- Assess first when you do not have enough data to determine the problem. Example: a post-operative client reports pain -- assess pain characteristics before medicating.
- Intervene first when the situation is life-threatening and the problem is clear. Example: a client is choking -- perform abdominal thrusts, do not assess further.
- If two answer choices are both assessments, choose the one most relevant to the presenting complaint.
- If two answer choices are both interventions, choose the one that addresses the most immediate threat to life.
5. Fluid and Electrolyte Balance
Fluid and electrolyte questions are common across multiple body systems. Key principles:
- Isotonic solutions (0.9% NS, Lactated Ringer's): expand intravascular volume without shifting fluid between compartments. Use for dehydration and blood loss.
- Hypotonic solutions (0.45% NS): fluid shifts into cells. Never give to clients with increased ICP or third-spacing.
- Hypertonic solutions (3% saline, D10W): fluid shifts out of cells. Administer via central line; monitor for fluid overload.
- Monitor I&O, daily weights (1 kg = approximately 1 liter of fluid), and lung sounds when managing fluid balance.
6. Pharmacology "Red Flag" Rules
These pharmacology rules are tested repeatedly:
- ACE inhibitors (ending in -pril): monitor for dry cough and angioedema. Check potassium -- can cause hyperkalemia.
- Beta-blockers (ending in -olol): monitor heart rate and blood pressure. Hold if HR below 60 bpm. Do not discontinue abruptly.
- Aminoglycosides (gentamicin, tobramycin): monitor trough levels, BUN/creatinine, and hearing (ototoxicity and nephrotoxicity).
- Heparin: monitor aPTT (therapeutic: 1.5-2.5x control). Antidote is protamine sulfate.
- Warfarin: monitor PT/INR. Antidote is vitamin K (phytonadione).
- Opioids: monitor respiratory rate. Hold if RR below 12/min. Antidote is naloxone (Narcan).
7. Pre- and Post-Procedure Nursing Actions
The NCLEX often asks what the nurse should do before or after a diagnostic procedure:
- Cardiac catheterization: post-procedure, monitor the insertion site for bleeding/hematoma, check distal pulses, keep the extremity straight for the prescribed time.
- Lumbar puncture: post-procedure, keep the client flat for the prescribed time to prevent spinal headache. Encourage fluid intake.
- Liver biopsy: position the client on the right side post-procedure to apply pressure to the puncture site. Monitor for hemorrhage.
- Bronchoscopy: NPO before; after, assess gag reflex return before offering fluids.
- Thyroidectomy: monitor for hemorrhage (check behind the neck), hypocalcemia (positive Chvostek's and Trousseau's signs), and laryngeal nerve damage (hoarseness).
8. Acid-Base Balance Interpretation
Use the ROME mnemonic to interpret ABG results:
- Respiratory = Opposite: pH and PaCO2 move in opposite directions (pH down, PaCO2 up = respiratory acidosis).
- Metabolic = Equal: pH and HCO3 move in the same direction (pH down, HCO3 down = metabolic acidosis).
Normal values: pH 7.35-7.45, PaCO2 35-45 mmHg, HCO3 22-26 mEq/L. Look at pH first to determine acidosis or alkalosis, then match to the component that explains the change. If the other component has also shifted, compensation is occurring.
